AUTHORIZATION TO FILE LETTER OF INTENT

I,                   , hereby authorize KineticEd and its representatives to act as my agent for the limited purpose of preparing and submitting a Letter of Intent to Establish a Home Education Program to the school superintendent of              County, Florida, on my behalf.

This authorization covers the following actions on my behalf:

(a) Preparing the Letter of Intent in compliance with Florida Statute §1002.41;
(b) Submitting the Letter of Intent to the appropriate county school district office;
(c) Receiving any acknowledgment or response from the school district related to this filing.

This authorization is limited in scope to the LOI filing described above and does not grant KineticEd authority to act on my behalf in any other matter. E-notarization, if required by the county, will be completed separately and a KineticEd representative will contact you to arrange this if needed.
